If you feel like you’re ready to enroll as a member of Seeds of Health, complete the form below and our doctor or staff will get in touch with you. At that time, we’ll schedule a Meet and Greet Consultation with Dr. Andrews to discuss the practice, membership requirements, and help you decide if Seeds of Health will be a good fit for you. If so, we’ll get you enrolled, request prior records, and set up your new patient appointment.
